Madison Police Respond To Scene of Fatal Accident on State Road 56

The investigation into this fatal crash is ongoing. 

(Madison, Ind.) – Officers with the Madison Police Department responded to a single-vehicle accident that occurred on State Road 56 near the entrance of Indiana Kentucky Electric Plant on the morning of August 21.

The initial investigation found that a westbound passenger car driven by Justin K. Cook, 34, of Madison left the travel portion of the roadway, striking a utility pole.

Officers and KDH-EMS rendered first aid at the scene before Cook was flown to the University of Louisville Hospital, where he later passed away.

Madison Police Patrolman Joe Gibson is leading the investigation, with the assistance of the Indiana State Police accident reconstructionist.

Madison Police Officers Stephan, Slygh, Melton, and Wehner assisted.  Clifty Six Fire & Rescue responded, along with fair Play Fire Company One.
